Home Accessibility Adaptation – This service makes changes to your home to help you live and move in your home safely and more easily. It can include changes like installing grab bars in your bathroom or a special toilet seat. It does not include major changes like new carpeting, roof repairs, plumbing systems, etc.
Individuals for whom it has been decided are eligible to receive long-term care services (LTC) through the Statewide Medicaid Managed Care (SMMC) Long-Term Care program will receive a welcome letter and brochure with information about the SMMC program and how to select a plan from the Agency for Health Care Administration (Agency). The letter includes:
- SMMC Plan Assignment(s) and Effective Date(s);
- Instructions for Changing Plans; and
- Rights to Change Plans; and
- A list of other LTC plans in your region.
To select a different plan, the individual must make a plan choice by the date in the letter. SMMC plans by region (view a map of regions and counties] ) are on the Agency’s Statewide Medicaid Managed Care website. Choice counselors are available by phone at 1-877-711-3662 to help recipients select a plan that best meets their needs. In-person visits are available by request for recipients with special needs. Recipients can also enroll online at: www.flmedicaidmanagedcare.com.
This system also offers the public access to a statewide database of local community resources, available on the internet or by calling the Elder Helpline at 1-800-963-5337.
The Brain and Spinal Cord Injury Program’s purpose is to provide all eligible residents who sustain a traumatic brain or spinal cord injury the opportunity to obtain the necessary services that will enable them to return to an appropriate level of functioning in their community. Our programs for eligible adult and pediatric clients provide direct case management and resource facilitation as primary services. This is accomplished by employing a statewide system of case managers and rehabilitation technicians. The program also employs regional managers who supervise staff in their region and who oversee the operation, development, and evaluation of the program’s services and supports at the local level.
Services provided to clients may include: case management, acute care, inpatient and outpatient rehabilitation, transitional living, assistive technology, home and vehicle modifications, nursing home transition facilitation; and long-term support for survivors and families through contractual agreements with community-based agencies. Once a client is accepted into the program they will develop a specialized care plan based on their specific injury and needs with the goal of achieving community reintegration.
The Division of Vocational Rehabilitation is the State of Florida’s largest employment program serving the needs of Floridians with disabilities. VR is charged with providing an array of employment supports and job placement assistance to eligible individuals with disabilities. These employment supports may include tuition assistance for colleges or vocational schools, books and supplies needed for school, assistive technology aids or devices, transportation services, occupational licenses, vehicle modifications, psychological counseling, medical services, and job placement assistance. Services are individualized and are based on an individual’s needs and employment goals. A map of all VR offices statewide is available on their website.
The Adult Protective Services Home and Community-Based Service Programs are fully encumbered at this time. Those seeking enrollment will be placed on a statewide waiting list for any and/or all programs for which they are determined eligible.
The Adult Protective Services Program supports ongoing case management and in-home services through Community-Based services (Chapter 410, F.S.). The Program Office sets guidelines, pursuant to applicable statutory and/or Federal Regulations, for in-home care for adults with disabilities in family-type living arrangements, such as private homes, as an alternative to institutional or nursing home care. Clients who qualify for these services are adults with disabilities aged 18 through 59 years with permanent physical or mental limitations that restrict their ability to perform normal activities of daily living and their capacity to live independently. Through case management and other in-home services, the program supports and maintains adults with disabilities’ independence and quality of life.
Disability Rights Florida was founded in 1977 as the state’s designated Protection and Advocacy (P&A) system for individuals with disabilities in the State of Florida. The P&A system exists to ensure the safety, wellbeing, and success of people with disabilities. P&As demand access and accountability in:
- health care
- education
- transportation
- voting
- housing
- employment, and within the juvenile and criminal justice systems.
Disability Rights Florida has access authority to any agency, institution, and organization that affects or involves people with disabilities. This is a power that other agencies do not have.
